About shoulder pain

Shoulder pain occurs frequently, which is why we see a number of patients with shoulder pain at the clinic. The symptoms are typically pain and reduced mobility in the shoulder. Sometimes the pain also radiates up towards the neck. Shoulder pain can be located in the front, back and side of the shoulder joint. We have great success treating shoulder pain and injuries in the shoulder. It is important to have the shoulder examined thoroughly, as there can be many causes of shoulder pain. In this context, the neck, elbow and hand should also be examined, as some of the causes may originate there. You can read more about our unique approach to treating shoulder pain further down.

Causes of shoulder pain

There can be many reasons for shoulder pain. It can be due to an acute injury, which typically occurs in connection with sports or other activities. The pain can also have arisen gradually and lasted for a long time. In the case of long-term shoulder pain, doctors typically have difficulty giving a specific diagnosis. In these cases, the pain is most often due to reduced strength in one or more shoulder muscles or reduced mobility in the elbow, shoulder, ribs or neck. Shoulder pain can also be due to overload of the tendons (tendinitis), impingement or inflammation of the bursae. In this case, it is essential to be thoroughly examined in the shoulder and upper body, which can help to find the underlying cause of the problem.

Treatment of shoulder pain

It is important that the treatment is carried out on the basis of a thorough examination of the individual patient. A thorough examination includes, among other things, testing the strength of all the muscles of the upper body and an assessment of the mobility of the shoulder, elbow, shoulder girdle and neck. The treatment must be specific, as a specific cause for the problem can often be found. The cause is typically hidden in the injury history in combination with the examination. At Copenhagen Physio, we aim to find the underlying cause. We can then specify the treatment, which typically aims to activate and strengthen the weak muscles using muscle activation and regain normal mobility in all joints.

Rotator cuff

The rotator cuff is a collective term for the four muscles (and tendons) that stabilize the shoulder joint. The muscles ensure that the joint head remains in the socket when you use your arm. The muscles are small and are best suited to stabilizing the joint. The challenges with the rotator cuff muscles typically arise if they have to compensate for the larger muscles that move the shoulder joint with great force. This causes the muscles and tendons of the rotator cuff to become overloaded and inflamed over time. It is therefore essential to identify the weak, large muscles around the shoulder if you want to treat the underlying cause of the problem. If the weak muscles are not strengthened, the exercises for the rotator cuff will have no effect.

Supraspinatus

The supraspinatus often becomes overused and inflamed. More precisely, it is the tendon that ends up becoming thickened, inflamed and pinched (impingement). The supraspinatus stabilizes the shoulder joint and move the shoulder up to the side (abduction). However, the dominant muscle responsible for this movement is the large deltoid muscle, which gives the shoulder its shape and contour. If the deltoid muscle is weak, the supraspinatus will be the second best muscle to abduct the shoulder joint. However, the supraspinatus is not designed to develop as much force as it requires, which is why it ends up being overused. Therefore, treatment of the supraspinatus tendon (in our opinion) should always start with strengthening the deltoid muscle if rehabilitation should have an effect.

Shoulder exercises

Shoulder exercises can be an important tool for becoming injury-free and pain-free, especially when it comes to shoulder rehabilitation. It is important to first make sure that the nerve activates the muscles you are trying to retrain. If the muscles are not activated, it can be very difficult to retrain, as the body will simply compensate, thereby training the "wrong" muscles. That is why we always start by activating the nerves and muscles. Once the nerves are activated, it is important that the exercises are precise and address the weak muscles. If you have previously tried a "standard program" for the shoulder without effect, the reason may be that the muscles are not activated. We can help you with that.

Frozen shoulder

Up to 5% of all citizens are affected by a frozen shoulder during their lifetime. The symptoms of a frozen shoulder are gradually increasing pain and significantly reduced mobility in all directions, both active and passive. Later in the course, the pain decreases, after which there is "only" reduced mobility. A frozen shoulder takes a long time to recover, and it may take 2-3 years before the shoulder is completely healthy again. The cause of a frozen shoulder is due to a shrinkage of the capsule. Why this happens is still unknown. That is why it is also incredibly difficult to treat. Sometimes a blockade can relieve some of the pain, but it does not solve the problem in the long term. When the pain allows it, you should start with movement exercises in order to prevent the loss of mobility as much as possible.

Other causes of shoulder pain

There may be other reasons for shoulder pain than those mentioned above. For example, the shoulder may dislocate (dislocation). This can/will cause damage to the ligaments and labrum, which will subsequently cause instability in the shoulder joint and pain. In addition, you may have torn fibers in many of the large muscles, such as the biceps and triceps. If you fall and are injured, you may have a broken bone, a broken collarbone, and torn ligaments and tendons. In the above cases, it is important to rehabilitate the shoulder correctly under the guidance of a specialized physiotherapist. In very rare cases, shoulder pain can be caused by nerve compression, arthritis, or an infection.

Physiotherapy is very effective in treating pain and injuries in the shoulder, if it is done correctly. Physiotherapy can be used to treat acute and overuse-related shoulder pain or for rehabilitation after a shoulder injury. In all cases, the goal is the same; To achieve full strength and free movement in and around the shoulder. In addition, it is important to understand the interaction of the shoulder with the elbow, shoulder girdle and neck. These joints can be important for the function of the shoulder, which is why they should also be examined and included in the treatment. We take pride in finding the underlying cause of the problem instead of just treating the symptoms. During treatment, we use muscle activation, joint mobilization, manipulations and exercises for the shoulder.
