About low back pain

Low back pain is characterized by pain in the lower back. It is estimated that 60-80% of the population will experience low back pain during their lifetime. During the past year, approximately 50% of the population will have suffered from low back pain. In 90% of cases, doctors cannot make a definitive diagnosis of low back pain. However, this does not mean that it cannot be treated. At Copenhagen Physio, we have great success in treating non-specific back pain. We focus on creating free movement and restoring full strength in the muscles around the lower back and pelvis. In this way, most low back pain disappears quickly, even if it has lasted for many years. For the same reason, we are often used as a second opinion.

Symptoms of low back pain

Symptoms of low back pain can vary in location, duration and intensity depending on how you challenge your lower back in your daily life. The pain can be stabbing, shooting, sharp or dull. The pain is often accompanied by stiffness around the lower back and pelvis. Below are listed the typical symptoms of low back pain:

  • Pain across the lower back, constant or varying.
  • Stiffness around the lower back and pelvis - pain during rotations.
  • Pain when standing and sitting down, as well as walking up stairs.
  • Radiating pain in the groin, buttock or leg.

Causes of low back pain

In up to 90% of cases, doctors cannot find an actual cause for low back pain. In these cases, it is our experience that the low back pain is due to reduced strength and activation of the buttocks, hip flexors or the deep back muscles in particular. Fortunately, we can treat this quickly and effectively. If these muscles are functioning properly, the vast majority of non-specific low back pain will disappear. However, low back pain can also be due to reduced mobility in the lower back, pelvis and hip, which can also be treated with the right techniques. The last 10% of cases can be due to nerve root involvement (due to, for example, herniated disc or spinal stenosis) and in very rare cases, cancer or inflammation.

Treatment of low back pain

We have great success in treating lower back pain. We have a different approach to treatment compared to other practitioners. We are thorough and also look at the interaction of the lower back with the foot, knee and hip, as this can be of great importance. It is important for us to restore strength in all the muscles around the lower back and pelvis. We often see that lower back pain is due to reduced activation and strength in the buttocks, hip flexors or the deep back muscle. When the strength and muscle activation are restored, most lower back pain disappears quickly and effectively. Our treatments also aim to create free mobility around the lower back, pelvis and hip, which is also of great importance.

Exercises for the lower back

Exercises for the lower back are of course important. However, the exercises only work if the muscles are correctly activated by the nervous system. It is the nerves that control and direct the muscles. If the nerve only sends signals at 50%, there will also only be 50% strength in the muscle. A large muscle can therefore be a weak muscle if it is not correctly activated. If the primary muscle for a given exercise is weak, the body will compensate by using the next best muscle. When we give exercises for the lower back, we first and foremost ensure that the muscle receives a full signal from the nerve. Afterwards we give specific exercises for the lower back. This approach has a very good effect on the treatment of lower back pain.

Treatment of a facet joint syndrome.

Facet joint syndrome

A stiff back is also called a back strain or a facet joint syndrome. A back strain is an acute injury that occurs suddenly during a lift or an "incorrect" movement. You can also wake up with a back strain. A back strain causes severe pain and limits mobility during normal movements, especially forward bending. A back strain is common, which is why we also see some of these patients at the clinic. Most often, we can treat the pain relatively quickly by restoring mobility in the lower back and ensuring that the muscles around the lower back function optimally. If some muscles are too weak, it will typically cause other muscles to tense up, which will "lock" the small joints in the back.

Only about 5% of all lower back pain is caused by a herniated disc in the lower back. Symptoms of a herniated disc in the lower back are typically radiating pain down the buttocks and legs. Lower back pain can also occur, but the radiating pain dominates. However, it is important to point out that a herniated disc does not necessarily cause pain or other symptoms. Many people have a herniated disc without having been bothered by it. Our approach to herniated discs focuses on restoring strength around the lower back and normalizing mobility in the lower back and pelvis. This approach is quite effective in reducing symptoms considerably. We cannot remove the herniated disc, but we can relieve the pain.

Other causes of low back pain

There are of course other causes of lower back pain than those described above. However, the lower back pain mentioned above is by far the most common we see in the clinic. Other causes include the following:

  • Scheuermann's disease.
  • Crooked back (scoliosis).
  • Spondylosis (osteoarthritis of the back).
  • Spinal stenosis (narrowing).
  • Spondylolisthesis (forward slippage).
  • Collapse of vertebrae due to osteoporosis.
  • Bectherew's disease.

Physiotherapy for low back pain

Physiotherapy is incredibly effective in treating low back pain if done correctly. We take pride in finding and treating the underlying cause of the low back pain. The underlying cause can be, due to reduced strength in the buttocks or reduced mobility in the hip, i.e. things that you don't necessarily think are related to the lower back. It is therefore important to get around the problem completely. The low back pain has arisen for a reason, and we need to find it. At the clinic we experience that the pain typically arises due to weakness in the buttocks, hip flexors or deep back muscles. Most low back pain disappears when these muscles function optimally and are correctly activated - even if the pain has lasted for years.
